Consideration of social determinants can be a barrier to case management or home care. This includes factors like income level, access to transportation and language proficiency. A lack of reliable transport may make it more difficult for an individual to go to their scheduled appointments. This can have a negative impact on overall health. Further complicating matters is the fact that those with lower educational levels and limited English proficiency might not be able to effectively navigate the healthcare system.
Furthermore, having a low income can also affect one’s ability to pay for necessary medical bills or medications which could exacerbate any existing conditions. Healthcare providers must consider these factors in order to provide adequate care for those with low incomes.
