Community nursing – week 5 – planning for community change
There are many steps involved in the planning of community health improvements. First, the community must identify and prioritise its health problems. You can do this by gathering data such as demographic information, surveys and the input of stakeholders. After identifying the health problems, evaluation and assessment should be done to identify which needs to be addressed. Next, set objectives. These should contain measurable goals for specific areas of health that will lead to desired outcomes. The plan must outline strategies to address the identified priorities. It may be used to implement policies that encourage healthy behaviours or offer resources and services that enhance access to healthcare.
